Anal fissure – A painful anal disease
An anal fissure is a small cut or crack or tear in the anus and anal canal. It may be acute or chronic. An anal fissure that lasts more than six weeks is called a chronic anal fissure.
a) Anal fissures are very common in infants, but they may occur at any age.
b) Fissure may occur when passing large or hard stools, straining during childbirth, or having diarrhea for a long time.
c) Decreased blood flow to the area
d) Too much tension in the sphincter muscles that control the anus.
e) The condition affects males and females equally. Anal fissures are also common in women after childbirth.
- f) In rare cases, an anal fissure may develop due to:
- anal cancer, HIV, tuberculosis, syphilis, herpes
The primary symptom of anal fissures is severe pain during and following bowel movements. Other symptoms that may occur are:
b) itching, and a
c) malodorous discharge.
A doctor can usually diagnose an anal fissure simply by visual inspection or examining the area around the anus and anal canal. However, they may want to perform a digital rectal exam to confirm the diagnosis.
Other medical tests that may be done include:
a) Anoscopy -- examination of the anus, anal canal, and lower rectum
b) Sigmoidoscopy -- examination of the lower part of large intestine
c) Biopsy -- removal of rectal tissue for examination
d) Colonoscopy -- examination of the colon
Most anal fissures don’t require treatment they heal on their own.However, certain tips can help promote healing and relieve uncomfortable symptoms.
a) Using over-the-counter stool softeners (laxatives).
b) Taking fiber supplements and increasing intake of fibrous foods, such as raw fruits and vegetables.
c) Taking warm baths called Sitz baths to relax the anal muscles, relieve irritation, and increase blood flow to the anorectal area.
d) Applying petroleum jelly or push coconut oil through disposable syringe to the anus to promote blood flow to the area and promote healing.
In Allopath an operation is an option if the fissure fails to heal despite the above treatments. It is also an option if you have fissures which happen again (recur). The usual operation is to make a small cut in the muscle around the anus (internal sphincterotomy). This permanently reduces the tone (pressure) around the anus and allows the fissure to heal. As with any operation, there is a risk of complications. After this operation, some people have poor control of wind (gas). A very small number have soiling of underclothes, or mild bowel incontinence.
Ayurveda way to heal:
If the fissure fails to heal despite the above treatments Ayurveda have a proper solution for it.
In Ayurveda mostly we adopt Ganesh kriya or Dilation process i.e. finger inserted in anal canal and by a proper method the fissure heal in 3-4 sitting (This process takes only 5-10 minutes, the sitting usually done in between 7-10 days) This procedure is having 70-80 % success rate.
If fissure is chronic or very deep generally in Ayurveda we adopt Ksharsutra procedure (The Ksharsutra is a medicated herbal thread which is ligated over the affected fissure or around the sentinel tag, this medicated thread has healing and cutting property through which the problem resolves completely in about 5-10 days) This is a minor procedure which is usually done as day care surgery (you will not usually have to stay in the hospital).The success rate with Ksharsutra is very high - at least 9 in 10 cases are cured.
Don’t ignore the disease:
Usually people tend to ignore the fissure disease due to shyness or social issue. If ignored Fissure may lead to a sentinel tag formation that hangs at anus margin. Sometimes a fissure may develop secondary bacterial infection that may lead to an abscess or Fistula formation which is quite painful and takes longer time to heal than Fissure itself. So, visit to a specialist doctor before it’s too late.
